The formulas (memorize these three)

Tablets / liquid

Amount = (Desired ÷ Have) × Quantity

"Want over have, times what it's in."

IV drip rate

gtt/min = (Volume × Drop factor) ÷ Time (min)

Drop factor is on the tubing box (10, 15, 20, or 60 gtt/mL).

IV pump rate

mL/hr = Volume (mL) ÷ Time (hr)

For an electronic pump.

Weight-based

Dose = mg/kg × weight (kg)

Then check it's a safe amount, and divide by doses/day if needed.
